Remote Patient Monitoring (RPM)
Virtual Care That Meets Patients at Home
Remote Patient Monitoring supports patients with diabetes and hypertension by delivering daily care touchpoints, health education, and biometric tracking—all from the comfort of home.
Remote Patient Monitoring (RPM)
- What is RPM?
Remote Patient Monitoring (RPM) allows care teams to monitor patients’ health outside of traditional clinical settings using connected medical devices. Patients may use devices to measure blood pressure, blood glucose, weight, oxygen saturation, and other health measures, with the data shared electronically with their care team.
By keeping track of patients between visits, RPM can help care teams identify changes in health earlier, support timely interventions, and manage chronic conditions more proactively. - Why use RPM?
RPM can help healthcare organizations:- Extend care beyond the clinic by staying connected with patients between appointments
- Identify changes earlier and provide timely follow-up when health data indicates a concern
- Support chronic disease management through ongoing monitoring and individualized care
- Engage patients in their care by helping them track and better understand their health
- Give care teams actionable information to support clinical decision-making
- Improve access to care for patients who may have difficulty attending frequent in-person visits
- Strengthen care coordination by incorporating patient-generated health data into the care plan
- Potentially reduce avoidable emergency department visits and hospitalizations through earlier intervention
- Best Practices
Successful RPM programs should:- Identify appropriate patients and conditions
- Establish clear data review, response, and escalation protocols
- Choose reliable, user-friendly technology
- Educate patients and provide technical support
- Define staff roles and workflows
- Integrate RPM data into the patient’s care plan
- Monitor patient engagement and program outcomes
- Maintain appropriate documentation, consent, billing, and compliance processes
