What is School-Based Telehealth?
School-based telehealth provides access to high-quality healthcare in the school setting, bringing care directly to students. With the assistance of the school nurse, a provider will provide care for the student via telehealth. This collaborative model helps students and parents avoid missed school and work to travel elsewhere to an appointment.
Please note that services vary by school to include acute care, point of care testing, chronic disease management, mental health, and group health education, etc. Contact your child’s school nurse to schedule an appointment and learn more about what services are offered.

School-Based Consent Forms
A school-based telehealth consent form is a document that authorizes the use of telehealth services for students within a school setting. It covers various elements to ensure that both parents or guardians and students understand and agree to the terms of the telehealth services being provided. This form ensures that all parties involved have a clear understanding of the telehealth process, and it establishes a framework for providing these services in a secure and informed manner.
Consent Form General Guidelines:
- Describe the services being offered and what the patient should expect during the encounter.
- If CMS billing is involved, include sections on:
- Financial Responsibility
- Medicare Patient Certification
- Agreement of Financial Responsibility for Non-Covered Services
- Release of Information for Alcohol or Substance Abuse
- HIPAA Compliance
- Address any potential risks, similar to the “Responsibility for Personal Belongings” section in the MUSC example consent form below.
